Is my Hospitals of Ontario Disability Income Program (HOODIP) insurance portable?

Good news! If you leave a position at a participating hospital and begin a job at another participating hospital within six months, your existing Group Long-term Disability (LTD), Group Life and Group Accidental Death and Dismemberment* (AD&D) insurance coverage is portable and can be transferred to your new employer.

If the gap between your old and new jobs is more than six months, you will be treated as a new applicant.

Who is responsible?

You are responsible for letting your new employer know that you have insurance coverage to transfer from your previous employer.

Your new employer is responsible for submitting the appropriate paperwork to the insurance provider and verifying any details of your previous coverage.

Make sure to let your employer know you need to transfer coverage as soon as possible, to avoid gaps in coverage or additional requirements!

What do I need to do?

You need to notify your new employer of your previous coverage. They will work with you to complete an Enrolment Form. If you received a Portability Form ((Form 3336-OHA) from your previous employer, make sure to provide a copy. If you don’t have a form, be prepared to provide information about your previous position, including the number of years employed and your termination date.

If you don’t have the necessary paperwork (Portability Form or a Wallet Certificate) with details of your former coverage, your new employer must contact your previous employer to verify it.

How long will it take to resume insurance?

If you leave a position with a participating employer and are employed by another participating employer within six months, you are eligible to resume insurance on the first day of work, provided you are actively at work on that date and enrollment is completed within 31 days of that date.

If you aren’t at work on your hire date, coverage won’t be effective until you return to active duty for:

  • Seven consecutively scheduled working days (full-time) OR
  • Ten consecutively scheduled working days (part-time)

If enrollment is completed more than 31 days after the first date of work, there will be a waiting period.

If you wait more than 31 days after your first day worked to inform your new employer, satisfactory medical evidence is required to complete the transfer of coverage. If evidence is declined or not submitted, the waiting period will also apply.
